Subject Number 046-908 Temporary Suspension of Dr. Leeland Jones’ Authorization to Treat Injured Workers Effective December 19, 2016

Subject Numbers Regarding Health Provider Authorizations

Authorization No. 176412-5W

December 22, 2016

Pursuant to Workers' Compensation Law Section 13-d(4), the Chair of the New York State Workers' Compensation Board has exercised his authority to temporarily suspend Dr. Leeland Jones’ authorization to treat injured workers within the New York State workers’ compensation system effective December 19, 2016. Dr. Jones’ last known business address is 3871 Harlem Road, Buffalo, New York 14215.

The Chair’s designee’s investigation has revealed that the New York State Department of Health, State Board for Professional Medical Conduct (BPMC) has issued Order # 16-394, wherein Dr. Jones agreed not to contest the allegations, including that he failed to adequately make an assessment of a diagnosis or detail a treatment plan for multiple patients, that he failed to complete or maintain a complete Mental Status Examination, failed to maintain complete medical records that accurately reflected the evaluation and treatment of multiple patients, that he failed to document any communication with other mental health providers, failed to obtain and/or monitor Lithium levels in a patient, changed a patient’s medication multiple times without adequate medical indication and/or without documenting such indication, and despite knowing a patient’s prior history and liking of cocaine, prescribed said patient an Amphetamine. This resulted in a censure and reprimand, and a two (2) year probationary period, during which Dr. Jones may practice medicine only when monitored by a licensed physician, board certified in an appropriate specialty (“Practice Monitor”).

Therefore, the Chair believes that Dr. Jones may be guilty of misconduct, and that such misconduct would detrimentally affect the quality of care provided to injured workers.

Dr. Jones is prohibited from rendering treatment and care to injured workers while his authorization is temporarily suspended. Reports submitted by Dr. Jones for services rendered prior to December 19, 2016 are valid, but are invalid for any services rendered on or after that date. Requests to cross-examine Dr. Jones for services rendered prior to December 19, 2016, should not be denied due to the suspension of his authorization.

This suspension remains in effect until further notice. Any questions regarding the authorized lists should be referred to the Medical Director’s Office at 1 (800) 781-2362.
