Authorization No. 168778-9W

April 11, 2016

Pursuant to Workers' Compensation Law Section 13-d(4), the Chair of the New York State Workers' Compensation Board has exercised his authority to temporarily suspend Dr. Moshe B. Mirilashvili’s authorization to treat injured workers within the New York State workers’ compensation system effective April 5, 2016. Dr. Mirilashvili's last known business addresses are 320 East Shore Road, Apartment 5-C, Great Neck, New York 11023, 2604 3rd Avenue, Apartment 5-C, Bronx, New York 10454, 600 Pine Hollow Road, Apartment 2-2B, East Norwich, New York 11732, and 90 Woodcrest Drive, Syosset, New York 11791.

The Chair’s designee’s investigation has revealed that on March 17, 2016, Dr. Mirilashvili was convicted in United States District Court for the Southern District of New York of distributing and conspiring to distribute a controlled substance (oxycodone) in violation of 21 U.S.C. § 841.

Therefore, the Chair believes that Dr. Mirilashvili may be guilty of misconduct, and that such misconduct would detrimentally affect the quality of care provided to injured workers.

Dr. Mirilashvili is prohibited from rendering treatment and care to injured workers while his authorization is temporarily suspended. Reports submitted by Dr. Mirilashvili for services rendered prior to April 5, 2016, are valid, but are invalid for any services rendered on or after that date. Requests to cross-examine Dr. Mirilashvili for services rendered prior to April 5, 2016, should not be denied due to the suspension of his authorization.

This suspension remains in effect until further notice. Any questions regarding the authorized lists should be referred to the Medical Director’s Office at 1 (800) 781-2362.
