Subject Numbers Regarding Health Provider Authorizations

September 18, 2009

Authorization No. 139943-5W

Please be advised that, by Determination and Order (BPMC No. 09-48), of a Hearing Committee of the New York State Department of Health, State Board for Professional Medical Conduct, effective March 31, 2009, the license to practice medicine of Brian Goldweber, M.D., in the State of New York was revoked. This action has been appealed to the Administrative Review Board of the New York State Department of Health, State Board for Professional Medical Conduct. Dr. Goldweber's license revocation remains in effect until the Review Board issues an order. Previously, by Interim Non-Disciplinary Order of Conditions, effective May 29, 2007, Dr. Goldweber was precluded from the practice of medicine, clinical or otherwise, in New York State and in any other jurisdiction where the practice of medicine is based solely upon his New York State medical license until final disposition of the investigation conducted by the New York State Office of Professional Medical Conduct, which culminated in the above-noted hearing and order. There is no current business address available for Dr. Goldweber.

Effective May 29, 2007, Dr. Goldweber is precluded from rendering medical treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Goldweber for treatment or examinations rendered prior to May 29, 2007, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Goldweber regarding treatment or examinations he rendered prior to May 29, 2007, should not be denied due to his preclusion from the practice of medicine or license revocation.
