DAVID P. NICHOLS, M.D. – Authorization Number 132972–1B

Please be advised that, effective November 21, 2008, Dr. David P. Nichols voluntarily resigned his authorization to conduct independent medical examinations of workers' compensation claimants. Dr. Nichols remains authorized to treat individuals who have suffered work–related injuries or illnesses under Authorization Number 132972–1W. His current address is 6000 Brockton Drive, Suite 109, Lockport, New York 14094–9273.

Dr. Nichols is prohibited from conducting independent medical examinations of individuals who have suffered work–related injuries or illnesses. Medical reports submitted by Dr. Nichols for examinations conducted prior to November 21, 2008, are valid, but are invalid for any such services on or after that date. Requests to cross–examine Dr. Nichols regarding examinations he conducted prior to November 21, 2008, should not be denied due to this resignation. In addition, testimony and/or depositions concerning independent medical examinations conducted through November 20, 2008, may be provided.

GORDON B. JOHN, D.P.M. – Authorization Number P03937–1W

Please be advised that, effective December 9, 2008, Dr. Gordon B. John voluntarily resigned his authorization to treat workers' compensation claimants. His license to practice podiatry in the State of New York is currently suspended, effective from January 23, 2008, to on or about January 22, 2010. His current address is 319 Utica Avenue, Brooklyn, New York 11213.

Dr. John is prohibited from rendering podiatric treatment and care to individuals who have suffered work–related injuries or illnesses. Medical reports submitted by Dr. John for treatment rendered prior to January 23, 2008, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross–examine Dr. John regarding treatment he rendered prior to January 23, 2008, should not be denied due to the suspension of his license or this resignation.
