Removal of Doctors' Names From The List of Physicians Authorized to Treat and/or Conduct Independent Medical Examinations of Workers' Compensation Claimants

ROBERT HAUBEN, M.D. (Authorization No. 095463–6W)

Please be advised that, by Determination and Order (BPMC No. 08–187), effective October 3, 2008, a Hearing Committee of the New York State Department of Health, State Board for Professional Medical Conduct (BPMC), revoked Dr. Robert Hauben's license to practice medicine in the State of New York.

Pursuant to Workers' Compensation Law Section 13–d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Hauben from the list of authorized physicians, effective November 10, 2008. Dr. Hauben's registered address is 351 Meeting House Lane, Suite G, Southampton, New York 11968

Dr. Hauben is prohibited from rendering medical or other treatment and care to individuals who have suffered work–related injuries or illnesses. Medical reports submitted by Dr. Hauben for treatment rendered prior to October 3, 2008, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross–examine Dr. Hauben regarding treatment he rendered prior to October 3, 2008, should not be denied due to the revocation of his medical license or his removal from the list of authorized physicians.

RAYMOND NISI, M.D. (Authorization No. 183813–5W)

Please be advised that, by Determination and Order (BPMC No. 08–58), effective September 30, 2008, the Administrative Review Board of the New York State Department of Health, State Board for Professional Medical Conduct (BPMC), revoked Dr. Raymond Nisi's license to practice medicine in the State of New York.

Pursuant to Workers' Compensation Law Section 13–d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Nisi from the list of authorized physicians, effective November 10, 2008. Dr. Nisi's registered address is 1200 Waters Place, Suite M101, Bronx, New York 10461.

Dr. Nisi is prohibited from rendering medical or other treatment and care to individuals who have suffered work–related injuries or illnesses. Medical reports submitted by Dr. Nisi for treatment rendered prior to September 30, 2008, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross–examine Dr. Nisi regarding treatment he rendered prior to September 30, 2008, should not be denied due to the revocation of his medical license or his removal from the list of authorized physicians.
