GHULAM S. MASOODI, M.D. (Authorization No. 130424–5W)

Please be advised that, effective March 31, 2008, Dr. Ghulam S. Masoodi permanently surrendered to the New York State Department of Health, State Board for Professional Medical Conduct, his license to practice medicine in the State of New York pursuant to New York State Public Health Law Section 230.13.

Effective June 18, 2008, Dr. Masoodi voluntarily resigned his authorization to treat workers' compensation claimants. His current address is 96 Oakview Drive, Buffalo, New York 14221-1420.

Dr. Masoodi is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Masoodi for treatment or examinations rendered prior to March 31, 2008, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Masoodi regarding treatment or examinations he rendered prior to March 31, 2008, should not be denied due to the surrender of his license or this resignation.

HANK ROSS, M.D. (Authorization No. 163233–0W)

Please be advised that, by Determination and Order (BPMC No. 06–86), effective August 21, 2006, the Administrative Review Board of the New York State Department of Health, State Board of Professional Medical Conduct (BPMC) affirmed the Hearing Committee's finding that Dr. Hank Ross was guilty of professional misconduct, and revoked his license to practice medicine in the State of New York, which penalty was appealed and denied.

Effective July 7, 2008, Dr. Ross voluntarily resigned his authorization to treat workers' compensation claimants. His current address is 161 Willis Avenue, Mineola, New York 11501.

Dr. Ross is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Ross for treatment or examinations rendered prior to August 21, 2006, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Ross regarding treatment or examinations he rendered prior to August 21, 2006, should not be denied due to the revocation of his license or this resignation.
