PERMANENT REVOCATION – DR. GAIL DEHART

Previously, the State Board for Professional Medical Conduct (BPMC) issued a Non-Practice Order, effective August 16, 2005, and, thereafter, Dr. Dehart surrendered her license to practice medicine to the BPMC, effective February 7, 2006. The Workers' Compensation Board (Board) temporarily suspended her authorization to treat workers' compensation claimants and to conduct independent medical examinations for one year, from October 26, 2005 through October 25, 2006, which was extended by commencement by the Board of permanent revocation proceedings on July 31, 2006.

Effective December 11, 2006, Dr. Dehart's authorization (Number 192269-9B) to treat workers' compensation claimants and to conduct independent medical examinations is permanently revoked pursuant to Workers' Compensation Law §13-d. Dr. Dehart's last known office address is located at 464 Peabody Road, Gouverneur, New York 13642.

Dr. Dehart is permanently prohibited from rendering medical or other treatment and care to or conducting independent medical examinations of individuals who have suffered work-related injuries or illnesses. Medical reports submitted for treatment or examination rendered prior to August 16, 2005 are valid, but are invalid for any treatment or examination rendered on or after that date. Requests to cross-examine Dr. Dehart regarding treatment or examination she rendered prior to August 16, 2005 should not be denied due to the surrender of her license or this revocation. In addition, testimony and/or depositions concerning independent medical examinations conducted through August 16, 2005 may be provided.

VOLUNTARY RESIGNATION – DR. KAUSHAL KIRIT VAIDYA

Please be advised that effective May 4, 2006, Kaushal Kirit Vaidya, M.D., surrendered his license to practice medicine in the State of New York to the State Board for Professional Medical Conduct. Effective November 24, 2006, Dr. Vaidya voluntarily resigned his authorization (Number 225768-1B) to treat and conduct independent medical examinations of workers' compensation claimants. Dr. Vaidya's last known business address is 11616 Jamaica Avenue, Richmond Hill, New York 11418-2456. His current address is 54 Quail Trail, Trumbull, Connecticut 06611.

Dr. Vaidya is prohibited from rendering medical or other treatment and care to and conducting independent medical examinations of individuals who have suffered work-related injuries or illnesses.

Medical reports submitted by Dr. Vaidya for treatment or examinations rendered prior to May 4, 2006, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Vaidya regarding treatment or examinations he rendered prior to the effective date of his license surrender should not be denied due to the surrender of his license or this resignation. In addition, testimony and/or depositions concerning independent medical examinations conducted through May 3, 2006 may be provided.
