PERMANENT REVOCATION - DR. ROGER J. BRICK, M.D.

Pursuant to a Summary Order of the Commissioner of Health, effective September 5, 2005, Roger J. Brick, M.D. was precluded from practicing medicine in the State of New York or any other jurisdiction where that practice was dependent on the valid New York State license to practice medicine or until the Summary Order was modified or vacated by the Commissioner of Health. The Summary Order remains in effect to date. Effective September 11, 2006, Dr. Brick's authorization (number 143817-5W) to treat workers' compensation claimants was permanently revoked pursuant to Workers' Compensation Law §13-d. Dr. Brick's last known office address is 50 East 42nd Street, Room 806, New York, New York 10017-5432. Dr. Brick is permanently prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries. Medical reports submitted for treatment rendered prior to September 5, 2005, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Brick regarding treatment he rendered prior to September 5, 2005, should not be denied due to this revocation.

VOLUNTARY RESIGNATION - DR. KENNETH MILLER, M.D.

Please be advised that effective August 24, 2006, Kenneth Miller, M.D. surrendered his license to practice medicine, and effective September 1, 2006, he voluntarily resigned his authorization (number 147634-0B, CIM) to treat and to conduct independent medical examinations of workers' compensation claimants. Dr. Miller's last known office address is 49 Breezewood Common, East Amherst , New York 14051-1424 .

Dr. Miller is prohibited from rendering medical or other treatment and care to and conducting independent medical examinations of individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Miller for treatment or examinations rendered prior to August 24, 2006 , are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Miller regarding treatment or examinations he rendered prior to the date he surrendered his license should not be denied due to this resignation. In addition, testimony and/or depositions concerning independent medical examinations conducted through August 24, 2006 , may be provided.

VOLUNTARY RESIGNATION (IME'S ONLY) - DR. RICHARD CARL DOBSON, M.D.

Please be advised that effective September 5, 2006 , Richard Carl Dobson, M.D. voluntarily resigned his authorization (number 135059-4B) to conduct independent medical examinations of workers' compensation claimants. Dr. Dobson's last known office address is 2005 Lyell Avenue, Suite 115 , Rochester , New York 14606-2325.

Dr. Dobson is prohibited from conducting independent medical examinations of individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Dobson for independent medical examinations rendered prior to September 5, 2006 , are valid, but are invalid for any independent medical examinations rendered on or after that date. Requests to cross-examine Dr. Dobson regarding independent medical examinations he rendered prior to the resignation's effective date should not be denied due to this resignation. In addition, testimony and/or depositions concerning independent medical examinations conducted through September 4, 2006 , may be provided.

Dr. Dobson is still authorized to treat workers' compensation claimants under authorization number 135059-4W.

VOLUNTARY RESIGNATION -- DR. RICHARD N. ADAMS, D.O.

Please be advised that effective February 24, 2006 , the State Board for Professional Medical Conduct suspended the license of Richard N. Adams, Jr., D.O. to practice medicine for one year, and effective June 26, 2006 , Dr. Adams voluntarily resigned his authorization (number 142256-7W) to treat workers' compensation claimants. Dr. Adams' last known office address is 56 Brinkerhoff Street , Plattsburgh , New York 12501.

Dr. Adams is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Adams for treatment or examinations rendered prior to February 24, 2006 , are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Adams regarding treatment or examinations he rendered prior to February 24, 2006 , should not be denied due to this resignation.
