Subject Numbers Regarding Health Provider Authorizations

Please be advised that effective October 26, 2005, Dr. Gail Dehart's authorization (number 192269-9B) to treat workers' compensation claimants and to conduct independent medical examinations has been temporarily suspended pursuant to Workers' Compensation Law §13-d(4). Dr. Dehart's office is located at 464 Peabody Road, Gouverneur, New York. The suspension is effective until October 25, 2006.

While this suspension is in effect, Dr. Dehart is prohibited from rendering medical or other treatment and care to or conducting independent medical examinations of individuals who have suffered work-related injuries or illnesses. Medical reports submitted for treatment or examinations rendered prior to October 26, 2005 are valid, but are invalid for any treatment or examination rendered on or after that date. Requests to cross-examine Dr. Dehart regarding treatment or examinations she rendered prior to the suspension's effective date should not be denied due to this suspension, and the physician is entitled to appropriate payment. In addition, testimony and/or depositions concerning independent medical examinations conducted through October 25, 2005 may be provided.
