Subject Numbers Regarding Health Provider Authorizations

Pursuant to Workers' Compensation Law Section 13-d(2)(a), the Chair of the New York State Workers' Compensation Board has exercised her authority to revoke Dr. Charles Bagley’s authorizations to treat injured workers and to perform independent medical examinations in the New York State workers’ compensation system effective September 7, 2018. Dr. Bagley's last known business addresses are:

Investigation has revealed that on or about August 30, 2018, Dr. Bagley pled guilty in the United States District Court for the Southern District of New York to one count of conspiracy to commit health care fraud.

Therefore, the Chair believes that Dr. Bagley may be guilty of misconduct, and that such misconduct would detrimentally affect the quality of care provided to injured workers.

Dr. Bagley is prohibited from rendering treatment and care to injured workers and from performing independent medical examinations. Reports submitted by Dr. Bagley for services rendered prior to September 7, 2018, are valid but are invalid for any services rendered on or after that date. Requests to cross-examine Dr. Bagley for services rendered prior to September 7, 2018, should not be denied due to the revocation of his authorizations.
