Please be advised that Philip Ashman, M.D., has resigned his authorization to treat injured workers in the New York State workers' compensation system effective August 3, 2012. Dr. Ashman's authorization to perform independent medical examinations was revoked effective July 18, 2012. Dr. Ashman's last known business address is 209 South Goodman Street, Rochester, New York, 14607.

Dr. Ashman is prohibited from rendering treatment and care to injured workers. Reports submitted by Dr. Ashman for treatment rendered prior to August 3, 2012, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Ashman for treatment rendered prior to August 3, 2012, should not be denied due to his voluntary resignation.

Dr. Ashman is prohibited from performing independent medical examinations. Reports submitted by Dr. Ashman for independent medical examinations rendered prior to July 18, 2012, are valid, but are invalid for any independent medical examinations rendered on or after that date. Requests to cross-examine Dr. Ashman for independent medical examinations rendered prior to July 18, 2012, should not be denied due to his revocation.
