Please be advised that Dr. Lawrence Lewis has resigned his authorization to treat and to perform independent medical examinations effective January 25, 2013. Furthermore, the New York State Department of Health's Office for Professional Medical Conduct (OPMC) has imposed a restriction on Dr. Lewis' license effective November 10, 2011. Dr. Lewis' last known business address is 344 East Main Street, Mount Kisco, New York, 10549.

Dr. Lewis is prohibited from providing treatment and care to injured workers. Reports submitted by Dr. Lewis for treatment performed prior to January 25, 2013, are valid, but are invalid for any treatment performed on or after that date. Requests to cross-examine Dr. Lewis for treatment performed prior to January 25, 2013, should not be denied due to this resignation.

Dr. Lewis is prohibited from performing independent medical examinations of injured workers. Reports submitted by Dr. Lewis for independent medical examinations performed prior to April 17, 2012, are valid, but are invalid for any independent medical examinations performed on or after that date. Requests to cross-examine Dr. Lewis for independent medical examinations performed prior to April 17, 2012, should not be denied due to the OPMC action.
