Please be advised that Orest Wasyliw, M.D., has resigned his authorization to treat injured workers in the New York State workers' compensation system effective October 24, 2012. Furthermore, Dr. Wasyliw's authorization to perform independent medical examinations has been removed effective October 24, 2012.

Dr. Wasyliw is prohibited from rendering treatment and care to injured workers and from performing independent medical examinations. Reports submitted by Dr. Wasyliw for services rendered prior to October 24, 2012, are valid, but are invalid for any services rendered on or after that date. Requests to cross-examine Dr. Wasyliw for services rendered prior to October 24, 2012 should not be denied due to his voluntary resignation or removal.
