Please be advised that, effective December 29, 2009, Alan H. Covey, M.D., permanently surrendered his license to practice medicine to the New York State Department of Health, Board for Professional Medical Conduct, pursuant to New York State Public Health Law Section 230.13. No current business address is available for Dr. Covey.

Dr. Covey is prohibited from rendering medical or other treatment and care to and conducting independent medical examinations of individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Covey for treatment rendered or examinations performed prior to December 29, 2009, are valid, but are invalid for any treatment rendered or examinations performed on or after that date. Requests to cross-examine Dr. Covey regarding treatment he rendered or examinations he performed prior to December 29, 2009, should not be denied due to the surrender of his license to practice medicine.

James Lash, M.D. (Authorization No. 079482-6W)

Please be advised that, effective December 4, 2009, James Lash, M.D., permanently surrendered his license to practice medicine to the New York State Department of Health, Board for Professional Medical Conduct, pursuant to New York State Public Health Law Section 230.13. No current business address is available for Dr. Lash.

Dr. Lash is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Lash for treatment rendered or examinations performed prior to December 4, 2009, are valid, but are invalid for any treatment rendered or examinations performed on or after that date.
