Please be advised that by Original Vote and Order Number 24625 of the New York State Board of Regents on behalf of the New York State Department of Education, Office of the Professions, the license of Thomas F. Miceli, D.C., to practice chiropractic was suspended for one month, effective November 2, 2009, to December 1, 2009. His last known business address is 33 Leeds Boulevard, Farmingville, New York 11738-1147.

During the period of actual suspension, Dr. Miceli was prohibited from rendering medical treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Miceli for treatment rendered prior to November 2, 2009, and after December 1, 2009, are valid, but are invalid for any treatment rendered on or through these dates. However, please note that per Subject No. 046-193, medical reports submitted for treatment rendered from May 29, 2007 through May 28, 2008, are also invalid due to a prior suspension. Requests to cross-examine Dr. Miceli regarding treatment he rendered prior to May 29, 2007, and after May 28, 2008 as well as prior to November 2, 2009, and after December 1, 2009, should not be denied due to the temporary suspension of his license to practice chiropractic.
