Please be advised that, effective August 15, 2009, Louis A. Perrotta, Jr., M.D., voluntarily resigned his authorization to treat individuals who have suffered work-related injuries or illnesses. Dr. Perrotta previously retired from the active practice of medicine in the State of New York. No current business address is available for Dr. Perrotta.

Dr. Perrotta is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Perrotta for treatment rendered prior to August 15, 2009, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Perrotta regarding treatment he rendered prior to August 15, 2009, should not be denied due to the resignation of his authorization.
