HARRY R. BLACK, M.D. (Authorization No. 119574-2W)

Please be advised that Harry R. Black, M.D., surrendered his license to practice medicine in the State of New York, effective March 12, 2009. No current business address is available for Dr. Black.

Dr. Black is prohibited from rendering medical treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Black for treatment rendered prior to March 12, 2009, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Black regarding treatment he rendered prior to March 12, 2009, should not be denied due to the surrender of his license.

ARTHUR M. MINKOFF, D.P.M. (Authorization No. P03104-8W)

Please be advised that Arthur M. Minkoff, D.P.M., surrendered his license to practice podiatry in the State of New York, effective February 18, 2009. No current business address is available for Dr. Minkoff.

Dr. Minkoff is prohibited from rendering podiatric treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Minkoff for treatment rendered prior to February 18, 2009, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Minkoff regarding treatment he rendered prior to February 18, 2009, should not be denied due to the surrender of his license.
