Authorization No. 161299-3W, formerly Authorization No. 161299-3B

Please be advised that, by Notice of Revocation, effective March 18, 2009, Dr. Dowd's authorization to render medical treatment and other care was revoked by the Chair pursuant to Workers' Compensation Law section 13-d. Previously, Dr. Dowd's medical license was limited by Consent Order (BPMC No. 06-298) entered into by him and adopted by the New York State Department of Health's Board for Professional Medical Conduct. By that Consent Order, Dr. Dowd was precluded from conducting independent medical examinations, effective December 26, 2006. Dr. Dowd's current address is 3771 Nesconset Highway, Suite 102A, Port Jefferson Station, New York 11720.

Dr. Dowd is prohibited from rendering medical treatment and care to and conducting independent medical examinations of individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Dowd for treatment rendered prior to March 18, 2009, are valid, but are invalid for any treatment on or after that date. Reports of independent medical examinations submitted by Dr. Dowd for examinations he conducted prior to December 26, 2006, are valid, but are invalid for any examinations on or after that date. Requests to cross-examine Dr. Dowd regarding treatment he rendered prior to March 18, 2009, or independent medical examinations he conducted prior to December 26, 2006, should not be denied due to the revocation of his authorization or limitation on his medical license.
