EMMA BENJAMIN, M.D. (Authorization No. 209190-8W)

Please be advised that, by Determination and Order (BPMC No. 08–17) effective February 11, 2008, a Hearing Committee of the New York State Department of Health, State Board for Professional Medical Conduct found Dr. Benjamin guilty of having been convicted in New York State Supreme Court, Queens County, of Falsifying Business Records and having been convicted in Criminal Court of the City of New York of Insurance Fraud. Previously, Dr. Benjamin's New York State medical license was summarily suspended by the New York State Commissioner of Health as of April 9, 2007.

Pursuant to Workers' Compensation Law Section 13–d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Benjamin from the list of authorized physicians, effective February 19, 2008. Dr. Benjamin's last known address is 7802 Flatlands Avenue, Brooklyn, New York 11236.

Dr. Benjamin is prohibited from rendering medical or other treatment and care to individuals who have suffered work–related injuries or illnesses. Medical reports submitted by Dr. Benjamin for treatment or examinations rendered prior to April 9, 2007, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross–examine Dr. Benjamin regarding treatment or examinations she rendered prior to April 9, 2007, should not be denied due to the suspension of her license or her removal from the list of authorized physicians.

RON LEV, M.D. (Authorization No. 224833-4W)

Please be advised that, by Determination and Order (BPMC No. 08–11) effective January 14, 2008, a Hearing Committee of the New York State Department of Health, State Board for Professional Medical Conduct found Dr. Lev guilty of having been convicted in New York State Supreme Court, New York County, of Attempted Assault in the Second Degree. Previously, Dr. Lev's New York State medical license was summarily suspended by the New York State Commissioner of Health as of November 20, 2007.

Pursuant to Workers' Compensation Law Section 13–d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Lev from the list of authorized physicians, effective February 15, 2008. Dr. Lev's current address is 150 West 56th Street, New York, New York 10019.
