VOLUNTARY RESIGNATION — YEVGENIY MARGULIS, Ph.D.

Please be advised that, by Order of the Board of Regents, New York State Education Department (No. 22827), Dr. Margulis' license to practice psychology in the State of New York was suspended for six (6) months, effective on or about December 5, 2006.

Effective March 16, 2007, Dr. Margulis voluntarily resigned his authorization (Number S14157-2B) to treat and conduct independent medical examinations of workers' compensation claimants. His current address is 2126 Benson Avenue, Apt. 1C, Brooklyn, New York 11214-5034.

Dr. Margulis is prohibited from rendering medical or other treatment and care to and conducting independent medical examinations of individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Margulis for treatment or examinations rendered prior to December 5, 2006 are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Margulis regarding treatment or examinations he rendered prior to December 5, 2006 should not be denied due to the suspension of his license or this resignation. In addition, testimony and/or depositions concerning independent medical examinations conducted through December 4, 2006 may be provided.

TEMPORARY SUSPENSION — JORGE ANTONIO RIVERO, M.D.

Please be advised that, effective March 19, 2007, through March 18, 2008, Dr. Rivero's authorization (Number 102460-3W) to treat workers' compensation claimants is suspended by the New York State Workers' Compensation Board. Dr. Rivero's current address is 1536 Rockaway Parkway, Brooklyn, New York 11236.

Dr. Rivero is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Rivero for treatment or examinations rendered prior to March 19, 2007 are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Rivero regarding treatment or examinations he rendered prior to March 19, 2007 should not be denied due to this suspension.
