TEMPORARY SUSPENSION - JEFFREY FICANO, D.O.

Please be advised that effective October 26, 2006, Dr. Jeffrey Ficano consented to a temporary suspension of his authorization (Number 211216-7W) to treat workers' compensation claimants pursuant to Workers' Compensation Law §13-d(4). Dr. Ficano's last known office address is Oneida Health Care Center, 321 Genesee Street, Oneida, New York 13421. The suspension is effective until September 30, 2007.

While the suspension is in effect, Dr. Ficano is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries. Medical reports submitted for treatment rendered prior to October 26, 2006 are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Ficano regarding treatment he rendered prior to October 26, 2006 should not be denied due to this suspension.

VOLUNTARY RESIGNATION - NAUM G. VAISMAN, M.D.

Please be advised that effective October 30, 2006, Dr. Naum Vaisman voluntarily resigned his authorization (Number 136522-0W) to treat workers' compensation claimants. His authorization had previously been temporarily suspended effective October 2, 2006 through October 1, 2007. Dr. Vaisman's current address is 840 East 8th Street, Apt. 4J, Brooklyn, New York 11230-2744.

Dr. Vaisman is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Vaisman for treatment rendered prior to October 2, 2006 are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Vaisman regarding treatment he rendered prior to October 2, 2006 should not be denied due to this resignation.

VOLUNTARY RESIGNATION - MANUEL G. SANGUILY, M.D.

Please be advised that effective October 5, 2006 and until December 31, 2006, the State Board for Professional Medical Conduct has limited Manuel G. Sanguily, M.D.'s license to practice medicine to prescribing or administering drugs to only those individuals with whom he has a qualifying medical relationship prior to October 5, 2006. Effective January 1, 2007, Dr. Sanguily is precluded from patient contact and any practice of medicine, clinical or otherwise. Effective November 3, 2006, Dr. Sanguily voluntarily resigned his authorization (Number 103155-8W) to treat workers' compensation claimants. Dr. Sanguily's last known office address is located at 4290 Broadway, Suite 2S, New York, New York 10033-3732, and his current address is 354 Fort Washington Avenue, New York, New York 10033.

Medical reports submitted by Dr. Sanguily for treatment rendered to workers' compensation patients prior to November 3, 2006 are valid provided there existed an established medical relationship with Dr. Sanguily prior to October 5, 2006, and the treatment was within the limitations of his license as set by the State Board for Professional Medical Conduct. Medical reports for any treatment rendered on or after November 3, 2006 are invalid. Requests to cross-examine Dr. Sanguily regarding treatment that he validly rendered to an established workers' compensation patient prior to November 3, 2006 should not be denied due to this resignation.

Any medical reports submitted by Dr. Sanguily for treatment rendered to a workers' compensation patient who has suffered work related injuries or illnesses and with whom Dr. Sanguily did not have an established medical relationship as of October 4, 2006 are invalid. Requests to cross-examine Dr. Sanguily regarding treatment he rendered after October 4, 2006 to a non-established workers' compensation patient should be denied due to the limitation of his license.

VOLUNTARY RESIGNATION - JOHN J. GELFAND, D.O.

Please be advised that, effective June 7, 2006, John J. Gelfand, D.O., surrendered his license to practice medicine in New York State or in any other jurisdiction where the practice of medicine is predicated on a valid New York State medical license to the State Board for Professional Medical Conduct.

Effective November 1, 2006, Dr. Gelfand voluntarily resigned his authorization (Number 151983-4W) to treat workers' compensation claimants. Dr. Gelfand's last two known office locations are: 1) 1 Elm Street, Suite 1C, Tuckahoe, New York 10707-3918; and 2) 1560 208th Place, Bayside, New York 11360-1122. His current address is 34 Garrison Road, Shady, New York 12409.

Dr. Gelfand is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Gelfand for treatment or examinations rendered prior to June 7, 2006 are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Gelfand regarding treatment or examinations he rendered prior to June 7, 2006 should not be denied due to the surrender of his license or this resignation.

VOLUNTARY RESIGNATION - WILLIAM TURSI, M.D.

Please be advised that effective August 18, 2006, the New York State Commissioner of Health ordered that William Tursi, M.D., may not practice medicine in New York State or in any other jurisdiction where the practice of medicine is predicated on a valid New York State medical license. Said Order remains in effect unless modified or vacated by the New York State Commissioner of Health.

Effective October 31, 2006, Dr. Tursi voluntarily resigned his authorization (Number 139729-8W) to treat workers' compensation claimants. Dr. Tursi's last known office address is located at 741 Jewett Avenue, Staten Island, New York 10314.

Dr. Tursi is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Tursi for treatment or examinations rendered prior to August 18, 2006 are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Tursi regarding treatment or examinations he rendered prior to August 18, 2006 should not be denied due to the Non-practice Order or this resignation.

PERMANENT REVOCATION - KHALIL AHMAD NAZIR, M.D.

Pursuant to the Determination and Order of the Hearing Committee of the State Board for Professional Medical Conduct (BPMC #06-07), effective January 25, 2006, Khalil Ahmad Nazir, M.D.'s license to practice medicine in the State of New York or any other jurisdiction where that practice was dependent on the valid New York State license to practice medicine was revoked.

Effective November 20, 2006, Dr. Nazir's authorization (Number 203260-5W) to treat workers' compensation claimants is permanently revoked pursuant to Workers' Compensation Law §13-d. Dr. Nazir's last known office address was located at 1659 Central Avenue, Suite 210, Albany, New York 12205. His current address is 2320 Egret Court, Rocklin, California 95675-4974.

Dr. Nazir is permanently prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries. Medical reports submitted for treatment rendered prior to January 25, 2006 are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Nazir regarding treatment he rendered prior to January 25, 2006 should not be denied due to the revocation of his license or his authorization.
