Subject Number 046-1424 Temporary Suspension of Dr. Sana M. Bloch

Subject Numbers Regarding Health Provider Authorizations

Authorization No. 131308-9W

July 8, 2021

Pursuant to Workers' Compensation Law Section 13-d(4), the Chair of the New York State Workers' Compensation Board has exercised her authority to temporarily suspend Dr. Sana M. Bloch's authorization to treat injured workers within the New York State workers' compensation system effective July 6, 2021. Dr. Bloch's last known business address is:

MedAlliance Medical
625 East Fordham Road
Bronx, New York 10458

The Chair's designee's investigation has revealed that, on or about April 28, 2021, the Workers' Compensation Board's Medical Director's Office (MDO) served a letter upon Dr. Bloch regarding a complaint that he refused to testify in a Workers' Compensation case regarding an injured worker for whom he rendered medical care. Dr. Bloch's response was due on or before May 13, 2021. To date, the MDO has not received a response to the previously served letter.

Therefore, the Chair believes that Dr. Bloch may be guilty of misconduct, and that such misconduct would detrimentally affect the quality of care provided to injured workers.

Dr. Bloch is prohibited from rendering treatment and care to injured workers, and from conducting reviews of records regarding variance requests pursuant to 12 NYCRR § 324.3(b)(2)(ii)(a), while his authorization is temporarily suspended. Reports submitted by Dr. Bloch for services rendered prior to July 6, 2021, are valid but are invalid for any services rendered on or after that date. Requests to cross-examine Dr. Bloch for services rendered prior to July 6, 2021, should not be denied due to the suspension of his authorization.

This suspension remains in effect until further notice. Any questions regarding the authorized lists should be referred to the Medical Director's Office at 1 (800) 781-2362.
