Pursuant to Workers' Compensation Law Section 13-d(2)(a), the Chair of the New York State Workers' Compensation Board has exercised her authority to remove Dr. Frank Craig Purpera, Jr.’s authorization to treat injured workers in the New York State workers’ compensation system effective December 29, 2019. Dr. Purpera's last known business address is:

The removal is based upon Determination and Order (BPMC 19-307), effective December 24, 2019, in which the Hearing Committee determined to revoke Dr. Purpera’s license to practice medicine in New York State, based on his felony convictions of 66 counts of obtaining a controlled substance by fraud in violation of Title 21 United States Code §§843(a)(3) and 843(d)(1), one count of omitting material information required to be kept in violation of Title 21 United States Code §§843(a)(4)(a) and (d)(1), and one count of false statement in violation of Title 18 United States Code §1001(a)(2).
Therefore, the Chair believes that Dr. Purpera may be guilty of misconduct, and that such misconduct would detrimentally affect the quality of care provided to injured workers.
Dr. Purpera is prohibited from rendering treatment and care to injured workers, and from conducting reviews of records regarding variance requests pursuant to 12 NYCRR § 324.3(b)(2)(ii)(a). Reports submitted by Dr. Purpera for services rendered prior to December 29, 2019, are valid, but are invalid for any services rendered on or after that date. Requests to cross-examine Dr. Purpera for services rendered prior to December 29, 2019, should not be denied due to the removal of his authorization.
