Subject Number 046-1023 Revocation of Dr. Rajendrakumar Patel

Subject Numbers Regarding Health Provider Authorizations

Authorization No. 151555-0W

February 5, 2018

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised her authority to revoke Dr. Rajendrakumar Patel’s authorization to treat injured workers within the New York State workers’ compensation system effective February 5, 2018. Dr. Patel's last known business address is:

5 Salina Street
Warsaw, New York 14569

The revocation is based upon Consent Order (BPMC No. 18-021), issued by the New York State Department of Health, State Board for Professional Medical Conduct (BPMC), effective February 6, 2018, wherein Dr. Patel admitted guilt to the Second Specification, including that he failed to maintain records for multiple patients which accurately reflected the evaluation and treatment of the patient, including failure to adequately assess medical history and symptoms upon presentation, failure to order adequate medical tests, failure to order appropriate consults, failure to adequately monitor blood gases and electrolytes, failure to adequately monitor lithium levels, sodium levels and thyroid functions, and failure to adequately document whether a patient was being followed by a psychiatrist. This resulted in a Censure and Reprimand, and a thirty-six (36) month probationary period, during which Dr. Patel may practice medicine only when monitored by a licensed physician, board certified in an appropriate specialty (“Practice Monitor”), and during which he must successfully complete a continuing education program as directed by the Office of Professional Medical Conduct.

Dr. Patel is prohibited from rendering treatment and care to injured workers. Reports submitted by Dr. Patel for services rendered prior to February 5, 2018 are valid, but are invalid for any services rendered on or after that date. Requests to cross-examine Dr. Patel for services rendered prior to February 5, 2018 should not be denied due to the revocation of his authorization.

Any questions regarding the authorized lists should be referred to the Medical Director’s Office at 1 (800) 781-2362.
