Please be advised that Dr. Veena Garyali has resigned her authorization to perform independent medical examinations in the New York State workers’ compensation system effective August 24, 2015. Dr. Garyali’s last known practice address is Apartment 1A, 75-58 113 Street, Forest Hills, New York 11375.

Dr. Garyali is prohibited from performing independent medical examinations of injured workers. Reports submitted by Dr. Garyali for independent medical examinations performed prior to August 24, 2015, are valid, but are invalid for independent medical examinations performed on or after that date. Requests to cross-examine Dr. Garyali for independent medical examinations performed prior to August 24, 2015, should not be denied due to her resignation.
