with respect to the effective date of the resignation.

The information regarding Dr. Tucciarone has been revised from the original release on June 9, 2015.

Please be advised that Dr. Michael Tucciarone has resigned his authorizations to treat injured workers and to perform independent medical examinations in the New York State workers’ compensation system effective June 1, 2015. Dr. Tucciarone’s last known practice address is 54 Dwight Avenue, Clinton, New York 13323.

Dr. Tucciarone is prohibited from rendering treatment and care to injured workers and from performing independent medical examinations. Reports submitted by Dr. Tucciarone for services rendered prior to June 1, 2015, are valid, but are invalid for any services rendered on or after that date. Requests to cross-examine Dr. Tucciarone for services rendered prior to June 1, 2015, should not be denied due to the resignation of his authorizations.

This Subject Number replaces the previously posted Subject Number 046-776. Any questions regarding the authorized lists should be referred to the Medical Director’s Office at 1 (800) 781-2362.
