Subject Numbers Regarding Health Provider Authorizations

Authorization No. 187275-3W

February 26, 2015

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to revoke Dr. Grace R. Maisel’s authorization to treat injured workers within the New York State workers’ compensation system effective February 23, 2015. Dr. Maisel's last known business address is 1552 Ralph Avenue, Brooklyn, NY 11236.

The revocation is based upon Consent Order (BPMC No. 14-300), issued by the New York State Department of Health, State Board for Professional Medical Conduct (BPMC), effective December 15, 2014.

Dr. Maisel could not successfully defend against at least one of the alleged acts of misconduct as stated in the consent agreement with the Department of Health dated December 14, 2014, resulting in a medical license limitation precluding her as an individual or as a professional corporation from evaluating, treating and/or billing patients whose medical services are to be paid through either no-fault or workers’ compensation. Dr. Maisel is also precluded from the practice of medicine until she has successfully completed a clinical competency assessment, after which her practice may be subject to further conditions.

Dr. Maisel is prohibited from rendering treatment and care to injured workers. Reports submitted by Dr. Maisel for services rendered prior to February 23, 2015, are valid, but are invalid for any services rendered on or after that date. Requests to cross-examine Dr. Maisel for services rendered prior to February 23, 2015, should not be denied due to the revocation of her authorization.

Any questions regarding the authorized lists should be referred to the Medical Director’s Office at 1 (800) 781-2362.
