Voluntary Resignation of Doctors' Authorizations to Treat or Conduct Independent Medical Examinations of Workers' Compensation Claimants

ROBERT L. STASIAK, D.C. (Authorization No. C04060–2B)

Effective June 6, 2008, Dr. Stasiak voluntarily resigned his authorization to conduct independent medical examinations of workers' compensation claimants. Dr. Stasiak remains authorized to render chiropractic treatment and care to individuals who have suffered work related injuries or illnesses. His current address is 1028 Abbott Road, Buffalo, New York 14220.

Dr. Stasiak is prohibited from conducting independent medical examinations of individuals who have suffered work–related injuries or illnesses. Medical reports submitted by Dr. Stasiak for examinations rendered prior to June 6, 2008, are valid, but are invalid for any examinations rendered on or after that date. Requests to cross–examine Dr. Stasiak regarding examinations he rendered prior to June 6, 2008, should not be denied due to this resignation.

MICHAEL ORENSTEIN, M.D. (Authorization No. 144524–6W)

Please be advised that, effective June 10, 2008, Michael Orenstein, M.D., voluntarily resigned his authorization to treat workers' compensation claimants. His current address is 692 County Highway 28, Cooperstown, New York 13326.

Dr. Orenstein is prohibited from rendering medical or other treatment and care to individuals who have suffered work–related injuries or illnesses. Medical reports submitted by Dr. Orenstein for treatment or examinations rendered prior to June 10, 2008, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross–examine Dr. Orenstein regarding treatment or examinations he rendered prior to June 10, 2008, should not be denied due to this resignation.

RICHARD GLISSON, D.O. (Authorization No. 187254–8W)

Please be advised that, effective June 7, 2008, Richard Glisson, D.O., voluntarily resigned his authorization (No. 187254–8W) to treat workers' compensation claimants. His current address is 5 Sunny Reach Drive, West Hartford, CT 06117.

Dr. Glisson is prohibited from rendering medical or other treatment and care to individuals who have suffered work–related injuries or illnesses. Medical reports submitted by Dr. Glisson for treatment or examinations rendered prior to June 7, 2008, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross–examine Dr. Glisson regarding treatment or examinations he rendered prior to June 7, 2008, should not be denied due to this resignation.
