Subject Numbers Regarding Health Provider Authorizations

TEMPORARY SUSPENSION – VLADIMIR MARGOLIN, M.D.

Please be advised that effective October 2, 2006, Dr. Vladimir Margolin's authorization (number 199935-8W) to treat workers' compensation claimants was temporarily suspended pursuant to Workers' Compensation Law §13-d(4). Dr. Margolin's last known office address is 10 Greenway, Oyster Bay, New York 11771. The suspension is effective until October 1, 2007.

While the suspension is in effect, Dr. Margolin is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries. Medical reports submitted for treatment rendered prior to October 2, 2006, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Margolin regarding treatment he rendered prior to October 2, 2006, should not be denied due to this suspension.

VOLUNTARY RESIGNATION – MICHAEL R. GLASS, M.D.

Please be advised that effective September 5, 2006, Dr. Michael Glass surrendered his license to practice medicine to the New York State Department of Health, State Board for Professional Medical Conduct; and effective September 18, 2006, Dr. Glass voluntarily resigned his authorization (number 101561-9W) to treat workers' compensation claimants. Dr. Glass's last known office address is 1345 Slaterville Road, Ithaca, New York 14850.

Dr. Glass is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Glass for treatment rendered prior to September 5, 2006, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Glass regarding treatment he rendered prior to the date he surrendered his license should not be denied.

VOLUNTARY RESIGNATION – JOSEPH F. ABOUMRAD, M.D.

Pursuant to a Summary Order of the Commissioner of Health, effective May 8, 2006, Joseph F. Aboumrad, M.D. was suspended from practicing medicine in the State of New York or any other jurisdiction where that practice was dependent on the valid New York State license to practice medicine. On June 29, 2006, Dr. Aboumrad surrendered his license to practice medicine to the State Board for Professional Medical Conduct; and effective August 26, 2006, Dr. Aboumrad voluntarily resigned his authorization (number 220175-4W) to treat workers' compensation claimants. Dr. Aboumrad's last known office address is 7900 Turin Road, Rome, New York 13440.

Dr. Aboumrad is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Aboumrad for treatment rendered prior to May 8, 2006, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Aboumrad regarding treatment he rendered prior to May 8, 2006, should not be denied.

PERMANENT REVOCATION – VICTOR CHEHEBAR, M.D.

As announced in Subject No. 046-143 dated July 13, 2005, Dr. Victor Chehebar's authorization to treat workers' compensation claimants was temporarily suspended pursuant to Workers' Compensation Law § 13-d(4). Said suspension was in effect from June 28, 2005, through June 27, 2006.

Please be advised that effective June 7, 2006, Dr. Chehebar's authorization (Number 158833-4W) to treat workers' compensation claimants was permanently revoked pursuant to Workers' Compensation Law §13-d. Dr. Chehebar's last known business address is 560 Northern Boulevard, Suite 104, Great Neck 11021.

Please note that Dr. Chehebar's license to treat any individual in New York State has been suspended by the State Board of Professional Medical Conduct since December 31, 2003, and will continue to be suspended through December 31, 2006.

VOLUNTARY RESIGNATION – SYED A. FAROOQ, M.D.

Effective August 17, 2006, pursuant to a Consent Order by the State Board for Professional Medical Conduct (BPMC No. 06-186), Dr. Syed Farooq's license to practice medicine in the State of New York was limited to preclude patient contact and any practice of medicine clinical or otherwise. On August 29, 2006, Dr. Farooq voluntarily resigned his authorization (Number 129137-6W) to treat workers' compensation claimants. Dr. Farooq's last known office address is 28 Halston Parkway, East Amherst, New York 14051-1842

Dr. Farooq is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Farooq for treatment rendered prior to August 17, 2006, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Farooq regarding treatment he rendered prior to August 17, 2006, should not be denied.

VOLUNTARY RESIGNATION – JONG-SUP RIM, M.D.

Pursuant to the Commissioner of Health, State Board for Professional Medical Conduct (BPMC No. 06-211), effective September 12, 2006, Dr. Jong-Sup Rim's license to practice medicine was limited to practicing only in a supervised setting where he has no responsibility for billing patients or third-party payors and he receives no compensation from payments made by patients or third-party payors.

Effective October 5, 2006, Dr. Rim voluntarily resigned his authorization (Number 123400-4W) to treat workers' compensation claimants. Dr. Rim's last known office address is 700 Mountain View Drive, Lewiston, New York 14092-1912.

Dr. Rim is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Rim for treatment rendered prior to September 12, 2006, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Rim regarding treatment he rendered prior September 12, 2006, should not be denied.

VOLUNTARY RESIGNATION – RAFIL ABDULLA DHAFIR, M.D.

Please be advised that effective February 6, 2006, the State Board for Professional Medical Conduct revoked Dr. Dhafir's license to practice medicine in the State of New York. Effective October 4, 2006, Dr. Dhafir voluntarily resigned his authorization (Number 142073-6B) to treat and to conduct independent medical examinations of workers' compensation claimants. Dr. Dhafir's last known address is P. O. Box 234, Fayetteville, New York 13066-0234.

Dr. Dhafir is prohibited from rendering medical or other treatment and care to and conducting independent medical examinations of individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Dhafir for treatment or examinations rendered prior to February 6, 2006, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Dhafir regarding treatment or examinations he rendered prior to the effective date of his license revocation should not be denied. In addition, testimony and/or depositions concerning independent medical examinations conducted through February 5, 2006, may be provided.

TEMPORARY SUSPENSION – ESTRELLA M. MARTINEZ, M.D.

Please be advised that effective August 29, 2006, the State Board for Professional Medical Conduct suspended the license of Estrella M. Martinez, M.D. for at least 18 months and until a modification order staying the suspension is issued by the State Board for Professional Medical Conduct.

Effective October 19, 2006, Dr. Martinez's authorization (Number 182125-5B) to treat and conduct independent medical examinations of workers' compensation claimants was temporarily suspended pursuant to Workers' Compensation Law – 13-d(4). Said suspension is in effect from October 19, 2006, through October 18, 2007. Dr. Martinez's last known business address is 553 Clinton Avenue, Albany, New York 12206-2738.

Dr. Martinez is prohibited from rendering medical or other treatment and care to and conducting independent medical examinations of individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Martinez for treatment or examinations rendered prior to August 29, 2006, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Martinez regarding treatment or examinations she rendered prior to August 29, 2006, should not be denied due to this suspension. In addition, testimony and/or depositions concerning independent medical examinations conducted through August 28, 2006, may be provided.
