Suspension/Revocation of Doctors' Authorizations to Treat Workers' Compensation Claimants

Date:  October 7, 2004

TEMPORARY SUSPENSION – DR. DON C. PIRRAGLIA, D.O.

Please be advised that effective August 31, 2004, Dr. Don C. Pirraglia's authorization (number 206652-0W) to treat workers' compensation claimants was temporarily suspended pursuant to Workers' Compensation Law §13-d(4). Dr. Pirraglia's office is located at 42-07 30th Avenue, Astoria, New York 11103. The suspension is effective until August 30, 2005.

Dr. Pirraglia is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses while this suspension is in effect. Medical reports submitted for treatment rendered prior to August 31, 2004 are valid, but are invalid for any treatment rendered after that date. Requests to cross-examine Dr. Pirraglia regarding treatment he rendered prior to the suspension's effective date should not be denied due to this suspension.

TEMPORARY SUSPENSION – DR. MARK FRANCIS ZONGRONE, M.D.

Please be advised that effective September 10, 2004, Dr. Mark Francis Zongrone's authorization (number 168016-4W) to treat workers' compensation claimants was temporarily suspended pursuant to Workers' Compensation Law §13-d(4). Dr. Zongrone's office is located at 1046 Erie Street, Utica, New York 13502. The suspension is effective until September 9, 2005.

Dr. Zongrone is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses while this suspension is in effect. Medical reports submitted for treatment rendered prior to September 10, 2004 are valid, but are invalid for any treatment rendered after that date. Requests to cross-examine Dr. Zongrone regarding treatment he rendered prior to the suspension's effective date should not be denied due to this suspension.

PERMANENT REVOCATION – DR. BARBARA GORDON-COHEN, D.O.

Please be advised that effective September 24, 2004, Dr. Barbara Gordon-Cohen's authorization (number 184468-7W) to treat workers' compensation claimants was permanently revoked pursuant to Workers' Compensation Law §13-d. Dr. Gordon-Cohen's office is located at 4 Boar Court, Suffern, New York 10901.

Dr. Gordon-Cohen is permanently prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries. Medical reports submitted for treatment rendered prior to September 24, 2004 are valid, but are invalid for any treatment rendered after that date. Requests to cross-examine Dr. Gordon-Cohen regarding treatment she rendered prior to the revocation's effective date should not be denied due to this revocation.
