This data may be incorporated into proprietary software systems used by parties which participate in the Workers' Compensation Board case resolution process.

For prescription drugs or medicines dispensed on or after July 7, 2008, the fee schedule is Average Wholesale Price (AWP) minus 12% for brand plus the dispensing fee and AWP minus 20% for generic plus the dispensing fee. The Medicaid fee schedule does not apply to prescription drugs or medicine dispensed on or after July 7, 2008.

File Structure:
The file is structured as variable-length fields and is delimited either by commas or tabs. The first record contains the field names.
Field Descriptions:-
NDC
- This is the National Drug Code and consists of 13 characters in a "00000-0000-00" format.

Type
- Indicates whether this drug is brand-name ('B') or generic ('G').
- Always one character in length.

Formulary Description
- Description of the drug and, usually, it's packaging format.
- Will not exceed 30 characters in length.
- The comma-delimited version wraps this field in double-quotes only when the field data contains a comma.

Price Effective Date
- First date on which the given price is effective for this drug.
- Field is 10 characters in length and format is MM/DD/YYYY.

Price End Date
- Last date on which the given price is effective for this drug.
- Field is 10 characters in length and format is MM/DD/YYYY.

Price
- Reimbursable amount for this drug which is in effect between the Effective and End dates (inclusive).
- Field is numeric with up to five decimal places.
