Subdivision (a) of section 325-6.15 of Title 12 NYCRR is hereby amended as follows:

1. The health insurer shall, together with its request for reimbursement, submit a non-refundable filing fee in the amount of [$75] $150 per request, for desk arbitrations, the arbitrator shall receive a fee of [$15] $40 per arbitration, payable by the dispute forum from the filing fees received by it.
