Please be advised that Dr. James R. Storms, III has resigned his authorization to treat injured workers in the New York State workers’ compensation system effective July 14, 2017. Dr. Storms’ last known practice addresses are 1486 Caton Road, Corning, New York 14830, and 352 East First Street, Corning, New York 14830.

Dr. Storms is prohibited from rendering treatment and care to injured workers. Reports submitted by Dr. Storms for services rendered prior to July 14, 2017, are valid, but are invalid for any services rendered on or after that date. Requests to cross-examine Dr. Storms for services rendered prior to July 14, 2017, should not be denied due to his resignation.
