Please be advised that, by Consent Order (BPMC No. 12-211), issued by the New York State Department of Health, State Board for Professional Medical Conduct (BPMC), effective October 22, 2012, Michael Palmeri, M.D., did not contest the charge of having been convicted in the City Court of White Plains, White Plains, New York of petit larceny; insurance fraud and filing a false instrument.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to revoke Dr. Palmeri's authorization to treat and to perform independent medical examinations effective December 26, 2012. Dr. Palmeri's authorization to provide services in the New York State workers' compensation system was previously suspended effective June 8, 2012. Dr. Palmeri's business address is 108 Van Guilder Avenue, New Rochelle, New York, 10801.

Dr. Palmeri is prohibited from rendering treatment and care to injured workers and from performing independent medical examinations. Reports submitted by Dr. Palmeri for services rendered prior to June 8, 2012, are valid, but are invalid for any services rendered on or after that date. Requests to cross-examine Dr. Palmeri for services rendered prior to June 8, 2012, should not be denied due to the revocation of his authorization.
