Subject Number 046-425 Limitation of Medical License of Jae Ook Park, M.D.

Subject Numbers Regarding Health Provider Authorizations

May 10, 2010

Authorization no. 130080-5B

Please be advised that, effective July 31, 2009, the New York State Education Department deemed the registration for Dr. Park's medical license to be expired. By Determination and Order (BPMC No. 10-63), effective April 27, 2010, a Hearing Committee of the New York State Department of Health, State Board for Professional Medical Conduct, sustained four specifications of professional misconduct against Jae Ook Park, M.D. As part of the penalty imposed, Dr. Park's medical license is restricted for one year barring him from performing workers' compensation or no-fault insurance examinations. Dr. Park must also complete continuing education requirements before resuming the practice of medicine in the State of New York. No current business address is available for Dr. Park.

Dr. Park is indefinitely prohibited from rendering medical or other treatment and care to or conducting independent medical examinations of individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Park for treatment rendered or examinations conducted prior to July 31, 2009, are valid, but are invalid for any treatment rendered or examinations conducted on or after that date, and until the one year restriction period has elapsed and/or Dr. Park has been cleared to resume the practice of medicine, whichever is later. Requests to cross-examine Dr. Park regarding treatment he rendered or examinations he conducted prior to July 31, 2009, should not be denied due to the expiration of his registration or restriction on his license to practice medicine.

Any questions regarding this matter should be referred to the Office of General Counsel at (518) 486-9564.
