Please be advised that, effective December 10, 2009, Richard J. Lewis, M.D., voluntarily resigned his authorization to render medical treatment and care to individuals who have suffered work-related injuries or illnesses. No current business address is available for Dr. Lewis.

Previously, by Subject Number 046-384, dated December 10, 2009, the Office of the Chair published notice of an Order for Non Practice of Medicine, effective November 20, 2009, issued by New York State Department of Health, State Board for Professional Medical Conduct, regarding the medical license of Richard Jay Lewis, M.D. This Order remains in effect.

Effective November 20, 2009, Dr. Lewis is precluded from rendering medical treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Lewis for treatment or examinations rendered prior to November 20, 2009, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Lewis regarding treatment or examinations he rendered prior to November 20, 2009, should not be denied due to the non practice of medicine order regarding his license to practice medicine nor the voluntary resignation of his authorization.
