ROBERT DURELL, M.D. (Authorization No. 082448-2W)

Please be advised that, by Consent Order (BPMC No. 09-138), as adopted by the New York State Department of Health, State Board for Professional Medical Conduct, effective July 29, 2009, Robert Durell, M.D., has agreed to never activate his registration or reapply for a license to practice medicine in the State of New York. No current business address is available for Dr. Durell.

Effective July 29, 2009, Dr. Durell is precluded from rendering medical treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Durell for treatment or examinations rendered prior to July 29, 2009, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Durell regarding treatment or examinations he rendered prior to July 29, 2009, should not be denied due to the inactive status of his license.

SUKOO LEE, M.D. (Authorization No. 111387-7W)

Please be advised that, by Consent Order (BPMC No. 99-202), as modified by Modification Order (BPMC No. 99-202), and adopted by the New York State Department of Health, State Board for Professional Medical Conduct, effective July 29, 2009, Sukoo Lee, M.D., has agreed to never activate his registration or reapply for a license to practice medicine in the State of New York. Previously, Dr. Lee's license registration was listed as inactive by the New York State Education Department as of March 31, 2001. No current business address is available for Dr. Lee.

Effective March 31, 2001, Dr. Lee is precluded from rendering medical treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Lee for treatment or examinations rendered prior to March 31, 2001, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Lee regarding treatment or examinations he rendered prior to March 31, 2001, should not be denied due to the inactive status of his license.
