MICHAEL CHAIT, M.D. (Authorization No. 184042-0W)

Please be advised that, by Surrender Order (BPMC No. 09-96), effective May 27, 2009, the New York State Department of Health, State Board for Professional Medical Conduct (BPMC), ordered the surrender of Dr. Chait's license to practice medicine in the State of New York.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Chait from the list of authorized physicians, effective June 3, 2009. No current business address is available for Dr. Chait.

Dr. Chait is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Chait for treatment rendered prior to May 27, 2009, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Chait regarding treatment he rendered prior to May 27, 2009, should not be denied due to the surrender of his medical license or his removal from the list of authorized physicians.

ASHRAF M. SAHAF, M.D. (Authorization No. 132722-0W)

Please be advised that, by Consent Order (BPMC No. 09-63), of the New York State Department of Health, State Board for Professional Medical Conduct (BPMC), effective April 16, 2009, Dr. Sahaf admitted guilt to the charge of negligence on more than one occasion.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Sahaf from the list of authorized physicians, effective June 3, 2009. Dr. Sahaf's address is 6030 Edward Avenue, Newfane, New York, 14108.

Dr. Sahaf is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Sahaf for treatment rendered prior to June 3, 2009, are valid, but are invalid for any treatment on or after that date. Requests to cross-examine Dr. Sahaf regarding treatment he rendered prior to June 3, 2009, should not be denied due to his removal from the list of authorized physicians.
