Andrew Ivanson, M.D. (Authorization No. 209134-6B)

Please be advised, effective April 22, 2009, Dr. Ivanson voluntarily resigned his authorization to treat and conduct independent medical examinations of workers' compensation claimants. No current business address is available for Dr. Ivanson.

Dr. Ivanson is prohibited from rendering medical or other treatment and care to and conducting independent medical examinations of individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Ivanson for treatment rendered or independent medical examinations conducted prior to April 22, 2009, are valid, but are invalid for any treatment rendered or independent medical examinations conducted on or after that date. Requests to cross-examine Dr. Ivanson regarding treatment he rendered or examinations he conducted prior to April 22, 2009, should not be denied due to his resignation of authorization.

Douglas W. Halliday, M.D. (Authorization No. 148892-3B)

Effective April 21, 2009, Dr. Halliday voluntarily resigned his authorization to treat and conduct independent medical examinations of workers' compensation claimants. His current business address is 4939 Brittonfield Parkway, Building B, Suite 210, East Syracuse, New York 13057.

Dr. Halliday is prohibited from rendering medical or other treatment and care to and conducting independent medical examinations of individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Halliday for treatment rendered or independent medical examinations conducted prior to April 21, 2009, are valid, but are invalid for any treatment rendered or independent medical examinations conducted on or after that date. Requests to cross-examine Dr. Halliday regarding treatment he rendered or examinations he conducted prior to April 21, 2009, should not be denied due to his resignation of authorization.

Inna Pustilnik, Ph.D. (Authorization No. S15189-4W)

Effective April 28, 2009, Inna Pustilnik, Ph.D., voluntarily resigned her authorization to treat workers' compensation claimants. No current business address is available for Dr. Pustilnik.

Dr. Pustilnik is prohibited from rendering psychological treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Pustilnik for treatment rendered prior to April 28, 2009, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Pustilnik regarding treatment she rendered prior to April 28, 2009, should not be denied due to her resignation of authorization.
