STEPHEN L. FRIEDES, M.D. (Authorization No. 160018-8W)

Effective January 13, 2009, Dr. Friedes voluntarily resigned his authorization to treat workers' compensation claimants. His current address is P.O. Box 1893, East Hampton, New York 11937.

Dr. Friedes is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Friedes for treatment rendered prior to January 13, 2009, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Friedes regarding treatment he rendered prior to January 13, 2009, should not be denied due to his resignation.

KAIZER T. KAMBLE, M.D. (Authorization No. 107286-7W)

BY Consent Order (BPMC No. 08-232), effective December 16, 2008, the New York State Department of Health, State Board for Professional Medical Conduct, limited Dr. Kamble's license to practice medicine in the State of New York to preclude all patient contact, and any and all practice of medicine, clinical or otherwise.

Effective January 16, 2009, Dr. Kamble voluntarily resigned his authorization to treat workers' compensation claimants. His current address is 16 Mar Del Way, Williamsville, New York 14221.

Dr. Kamble is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Kamble for treatment rendered prior to December 16, 2008, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Kamble regarding treatment he rendered prior to December 16, 2008, should not be denied due to his resignation.
