CARMEN A. D'ANGELO, M.D. (Authorization No. 163205–8W)

Please be advised that, by Determination and Order (BPMC No. 08–137), effective August 4, 2008, a hearing Committee of the New York State Department of Health, State Board for Professional Medical Conduct (BPMC), revoked Dr. Carmen A. D'Angelo's license to practice medicine in the State of New York.

Pursuant to Workers' Compensation Law Section 13–d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. D'Angelo from the list of authorized physicians, effective October 10, 2008. Dr. D'Angelo's current address is 200 Totem Trail, Rochester, New York 14617.

Dr. D'Angelo is prohibited from rendering medical or other treatment and care to individuals who have suffered work–related injuries or illnesses. Medical reports submitted by Dr. D'Angelo for treatment rendered prior to August 4, 2008, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross–examine Dr. D'Angelo regarding treatment he rendered prior to August 4, 2008, should not be denied due to the revocation of his medical license or his removal from the list of authorized physicians.

TORIN JONATHAN FINVER, M.D. (Authorization No. 200101–4W)

Please be advised that, by Consent Order (BPMC No. 08–141), effective August 8, 2008, the New York State Department of Health, State Board for Professional Medical Conduct (BPMC), suspended Dr. Torin Jonathan Finver's license to practice medicine in the State of New York for at least twelve months. Dr. Finver did not contest the charge of being a habitual user of alcohol and/or drugs.

Pursuant to Workers' Compensation Law Section 13–d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Finver from the list of authorized physicians, effective October 10, 2008. Dr. Finver's current address is 140 E. King Road, Ithaca, New York 14850.

Dr. Finver is prohibited from rendering medical or other treatment and care to individuals who have suffered work–related injuries or illnesses. Medical reports submitted by Dr. Finver for treatment rendered prior to August 8, 2008, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross–examine Dr. Finver regarding treatment he rendered prior to August 8, 2008, should not be denied due to the suspension of his medical license or his removal from the list of authorized physicians.

FRANK A. ZIMBA, M.D. (Authorization No. 197137–3W)

Please be advised that, by Consent Order (BPMC No. 08–164), effective September 16, 2008, the New York State Department of Health, State Board for Professional Medical Conduct (BPMC), adopted Dr. Frank A. Zimba's application for Consent Agreement and Order, wherein he admitted guilt to the charge of negligence on more than one occasion.

Pursuant to Workers' Compensation Law Section 13–d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Zimba from the list of authorized physicians, effective October 9, 2008. Dr. Zimba's current address is 207 Foote Avenue, Suite 330, Jamestown, New York 14701.

Dr. Zimba is prohibited from rendering medical or other treatment and care to individuals who have suffered work–related injuries or illnesses. Medical reports submitted by Dr. Zimba for treatment rendered prior to October 9, 2008, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross–examine Dr. Zimba regarding treatment he rendered prior to October 9, 2008, should not be denied due to his removal from the list of authorized physicians.
