One of the goals of the Workers' Compensation Reform initiative is to enhance the service the Board provides to injured workers, employers and stakeholders. A critical element of this effort is the redesign of forms the Board uses to gather information about a claim. The goal of the forms redesign effort is to develop more user-friendly forms while collecting additional information needed to more quickly process and resolve claims, including claims that have been controverted.
On September 16th the Board posted a revised Doctor's Initial Report (Form C-4) and a new Doctor's Progress Report (Form C-4.2) on its website for medical providers to begin using by January 1, 2009. In addition, the board will be releasing an electronic narrative version of these two forms (Form EC-4NARR) by January 1, 2009. The EC-4NARR contains significantly fewer questions than Forms C-4 and C-4.2. The doctor must include a narrative attachment with the form which will provide all the additional information which otherwise would be provided on the other forms. To assist the doctor, Form EC-4NARR includes a detailed list of required information that must be included in the narrative attachment.
Form EC-4NARR will only be accepted by the Board through electronic submission. The Board will accept the EC-4NARR as an equivalent to Forms C-4 or C-4.2 if it includes a narrative attachment and is filed electronically with the Board.
Form EC-4NARR will be available in a new Adobe format which will include features to assist in completing and electronically submitting the form. In addition the Board is prepared to work with providers, practice management software vendors and third parties to accept EC-4NARR forms through electronic batch submissions.
The Board is expecting to begin receiving the EC-4NARR forms in the near future and has posted a copy of this form on our web site for review.
