WILLIAM J. HEIDENBERG, M.D. (Authorization No. 086328–2B)

Please be advised that, according to a March 8, 2006, statement, Dr. William J. Heidenberg stated he is no longer engaged in the active practice of medicine.

Effective September 12, 2008, Dr. Heidenberg voluntarily resigned his authorization to treat and conduct independent medical examinations of workers' compensation claimants. His current address is 82 Soundview Avenue, White Plains, New York 10606.

Dr. Heidenberg is prohibited from rendering medical or other treatment and care to and conducting independent medical examinations of individuals who have suffered work–related injuries or illnesses. Medical reports submitted by Dr. Heidenberg for treatment or examinations rendered prior to March 8, 2006, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross–examine Dr. Heidenberg regarding treatment or examinations he rendered prior to March 8, 2006, should not be denied due to his inactive status or this resignation. In addition, testimony and/or depositions concerning independent medical examinations conducted through March 7, 2006, may be provided.

MORTEZA NAGHIBI, M.D. (Authorization No. 142078–5B)

Please be advised that, effective September 15, 2008, Dr. Morteza Naghibi, through his Attorney–in–Fact, Freshteh Salehi, voluntarily resigned his authorization to treat and conduct independent medical examinations of workers' compensation claimants. His current address is 6 Fenway Court, Loudonville, New York 12211.

Dr. Naghibi is prohibited from rendering medical or other treatment and care to and conducting independent medical examinations of individuals who have suffered work–related injuries or illnesses. Medical reports submitted by Dr. Naghibi for treatment or examinations rendered prior to September 15, 2008, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross–examine Dr. Naghibi regarding treatment or examinations he rendered prior to September 15, 2008, should not be denied due to this resignation. In addition, testimony and/or depositions concerning independent medical examinations conducted through September 14, 2008, may be provided.

ANTHONY J. RELLA, M.D. (Authorization No. 084323–5B)

Please be advised that Dr. Anthony J. Rella is retired from the practice of medicine.

Effective September 16, 2008, Dr. Rella voluntarily resigned his authorization to treat and conduct independent medical examinations of workers' compensation claimants. His current address is 6 Springhouse Way, Sterlington, New York 10974.

Dr. Rella is prohibited from rendering medical or other treatment and care to and conducting independent medical examinations of individuals who have suffered work–related injuries or illnesses. Medical reports submitted by Dr. Rella for treatment or examinations rendered prior to September 16, 2008, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross–examine Dr. Rella regarding treatment or examinations he rendered prior to September 16, 2008, should not be denied due to his retirement or this resignation. In addition, testimony and/or depositions concerning independent medical examinations conducted through September 15, 2008, may be provided.
