Revised Forms C-7, Notice That Right to Compensation is Controverted,
and PH-16.2, Pre-hearing Conference Statement

The recent adoption of the Indexing and Controverted Claims regulations impacts how the Board handles workers' compensation claims that have been controverted. In conjunction with these regulations, the Board is implementing revised versions of Form C-7, Notice That Right to Compensation is Controverted, and Form PH-16.2, Pre-Hearing Conference Statement. The forms were modified to capture information that must be included as required by the new regulations.

The recently adopted regulations regarding the Controverted Claims Process will be effective on November 3rd 2008, at which time all parties are required to begin using the new versions of Forms C-7 and PH-16.2. In order for parties to begin converting their processes to the new versions the Board had provided the updated Forms C-7 and PH-16.2 on the WCB web site. The electronic submission of Form C-7 will be updated with the new format on November 3rd.

As per the regulation, Form PH-16.2 is to be filed with the Board ten days prior to the Pre-Hearing Conference.
