Date: August 4, 2008

JATINDER SINGH BAKSHI, M.D. (Authorization No. 196731-4W)

Please be advised that, by Determination and Order (BPMC No. 08-22), effective June 30, 2008, the Administrative Review Board (ARB) of the New York State Department of Health, State Board for Professional Medical Conduct (BPMC), affirmed the February 7, 2008, determination of a BPMC Hearing Committee finding Dr. Jatinder Singh Bakshi guilty of professional misconduct. The ARB limited Dr. Bakshi's license to allow him to practice medicine only in a government licensed or operated health care facility.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Bakshi from the list of authorized physicians, effective July 17, 2008. Dr. Bakshi's current address is 21315 33rd Road, Bayside, New York 11361.

Dr. Bakshi is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Bakshi for treatment rendered prior to June 30, 2008, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Bakshi regarding treatment he rendered prior to June 30, 2008, should not be denied due to the limitation of his medical license or his removal from the list of authorized physicians.

Please be advised that, by Determination and Order (BPMC No. 08-116), effective July 3, 2008, a Hearing Committee of the New York State Department of Health, State Board for Professional Medical Conduct (BPMC), found Dr. Jan Arnett guilty of professional misconduct and revoked his license to practice medicine in the State of New York, or in any other jurisdiction where that practice is predicated on a valid New York State license to practice medicine.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Arnett from the list of authorized physicians, effective July 17, 2008. Dr. Arnett's current address is 4207 30th Avenue, Astoria, New York 11103.

Dr. Arnett is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Arnett for treatment rendered prior to July 3, 2008, are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Arnett regarding treatment he rendered prior to July 3, 2008, should not be denied due to the revocation of his medical license or his removal from the list of authorized physicians.
