Subject Numbers Regarding Health Provider Authorizations

June 17, 2008

STEPHEN B. RAVITZ, D.P.M. (Authorization No. P04798–6B)

Please be advised that, by Order (Vote and Order No. 23321), effective September 19, 2007, the Board of Regents of the University of the State of New York, on behalf of the State Education Department, adopted Dr. Ravitz's application for suspension of his license to practice as a podiatrist in the State of New York, which suspension was stayed. Dr. Ravitz was placed on probation for two years.

Effective May 23, 2008, Dr. Ravitz voluntarily resigned his authorization to conduct independent medical examinations of workers' compensation claimants. Dr. Ravitz maintains his authorization to treat injured or ill workers. His current address is 63 Main Street, Kings Park, New York 11754.

Dr. Ravitz is prohibited from conducting independent medical examinations of individuals who have suffered work–related injuries or illnesses. Medical reports submitted by Dr. Ravitz for independent medical examinations rendered prior to May 23, 2008, are valid, but are invalid for any independent medical examinations rendered on or after that date. Requests to cross–examine Dr. Ravitz regarding independent medical examinations he rendered prior to May 23, 2008, should not be denied due to this resignation.

Any questions regarding this matter should be referred to the Office of General Counsel at (518) 486-9564.
