PERMANENT REVOCATION: ERNEST ABELES, M.D.

Please be advised that, by Consent Order (BPMC No. 06-161), effective July 21, 2006, the New York State Department of Health, State Board for Professional Medical Conduct limited Ernest Abeles, M.D.'s license to practice medicine to preclude patient contact and any practice of medicine, clinical or otherwise, except in his current practice of conducting independent medical examinations for the Social Security Administration and for insurance carriers.

Effective October 25, 2006, Dr. Abeles' authorization (Number 091374-9B) to treat workers' compensation claimants and to conduct independent medical examinations of workers' compensation claimants was temporarily suspended pursuant to Workers' Compensation Law §13-d(4). Said suspension was in effect from October 25, 2006, through October 24, 2007, and was extended indefinitely by commencement of permanent revocation proceedings on July 6, 2007.

Effective January 18, 2008, Dr. Abeles' authorization to treat workers' compensation claimants and conduct independent medical examinations was permanently revoked pursuant to Workers' Compensation Law §13-d. Dr. Abeles' current address is 351 East 19th Street, New York, New York 10003.

Dr. Abeles is permanently prohibited from rendering medical or other treatment and care to or conducting independent medical examinations of individuals who have suffered work-related injuries or illnesses. Medical reports submitted for treatment or examination rendered prior to October 25, 2006, are valid, but are invalid for any treatment or examination rendered on or after that date. Requests to cross-examine Dr. Abeles regarding treatment or examination he rendered prior to October 25, 2006, should not be denied due to the suspension of his license or this revocation. In addition, testimony and/or depositions concerning independent medical examinations conducted through October 24, 2006, may be provided.

VOLUNTARY RESIGNATION – CHRISTOPHER GREEN, D.C.

Please be advised that, effective January 23, 2008, Christopher J. Green, D.C., voluntarily resigned his authorization (No. C07096-3W) to treat workers' compensation claimants. His current address is 2927 Bayview Avenue, Wantagh, New York 11793.

Dr. Green is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Green for treatment or examinations rendered prior to January 23, 2008, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Green regarding treatment or examinations he rendered prior to January 23, 2008, should not be denied due to this resignation.
