DENNIS U. CARBONELL, M.D. (Authorization No. 128949-5W)

Please be advised that, by Consent Order (BPMC No. 07-275) of the New York State Department of Health, State Board for Professional Medical Conduct, effective December 20, 2007, Dennis U. Carbonell, M.D.'s license to practice medicine in the State of New York, or in any other jurisdiction where that practice is predicated on a valid New York State license to practice medicine, was limited to prohibit him from prescribing, dispensing or administering controlled substances.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Carbonell from the list of authorized physicians, effective January 25, 2008. Dr. Carbonell's current address is 18 Eastwood Boulevard, Centereach, New York 11720.

Dr. Carbonell is prohibited from rendering medical or other treatment and care to individuals who have suffered work–related injuries or illnesses. Medical reports submitted by Dr. Carbonell for treatment or examinations rendered prior to January 25, 2008, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross–examine Dr. Carbonell regarding treatment or examinations he rendered prior to January 25, 2008, should not be denied due to the limitation of his license or his removal from the list of authorized physicians.

ROBERT F. ALTSCHULER, M.D. (Authorization No. 136057-7W)

Please be advised that, by Determination and Order (BPMC No. 07-264), effective December 6, 2007, a Hearing Committee of the New York State Department of Health, State Board for Professional Medical Conduct, sustained the charge that Robert F. Altschuler, M.D. was convicted in Vestal Town Court, Criminal Part, Broome County, New York, of driving while intoxicated.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Altschuler from the list of authorized physicians, effective January 29, 2008. Dr. Altschuler's current office address is Susquehanna Anesthesia Affiliate, 156 Corliss Avenue, Johnson City, New York 13790.
