JAMES REARDON, M.D. (Authorization No. 096607-7W)

Please be advised that, by Surrender Order (BPMC No. 07-259) of the New York State Department of Health, State Board for Professional Medical Conduct, effective December 5, 2007, Dr. James Reardon surrendered his license to practice medicine in the State of New York, or in any other jurisdiction where that practice is predicated on a valid New York State license to practice medicine.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Reardon from the list of authorized physicians, effective January 2, 2008. Dr. Reardon's former address is 737 Park Avenue, New York, New York 10021. No forwarding address information is available.

Dr. Reardon is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Reardon for treatment or examinations rendered prior to December 5, 2007, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Reardon regarding treatment or examinations he rendered prior to December 5, 2007, should not be denied due to the surrender of his license or his removal from the list of authorized physicians.

CLIFTON SCOTT THOMPSON, D.O. (Authorization No. 218959-5W)

Please be advised that, by Consent Order (BPMC No. 07-263) of the New York State Department of Health, State Board for Professional Medical Conduct (BPMC), effective December 5, 2007, Dr. Clifton Scott Thompson's license to practice osteopathic medicine in the State of New York, or in any other jurisdiction where that practice is predicated on a valid New York State license to practice osteopathic medicine, was suspended for not less than 3 months and until a modification order staying the suspension is issued by the BPMC.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Thompson from the list of authorized physicians, effective January 14, 2008. Dr. Thompson's current address is 91 Gordon Street, Jamestown, New York 14701.

Dr. Thompson is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Thompson for treatment or examinations rendered prior to December 5, 2007, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Thompson regarding treatment or examinations he rendered prior to December 5, 2007, should not be denied due to the suspension of his license or his removal from the list of authorized physicians.

HARRY L. WACHEN, M.D. (Authorization No. 080961-6B)

Please be advised that, by Consent Order (BPMC No. 07-261) of the New York State Department of Health, State Board for Professional Medical Conduct (BPMC), effective December 5, 2007, Dr. Harry L. Wachen's license to practice medicine has been limited to preclude the practice of medicine in New York State, clinical or otherwise.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Wachen from the list of authorized physicians, effective January 14, 2008. Dr. Wachen's current address is 76 Arleigh Road, Great Neck, New York 11021.

Dr. Wachen is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Wachen for treatment or examinations rendered prior to December 5, 2007, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Wachen regarding treatment or examinations he rendered prior to December 5, 2007, should not be denied due to the limitation of his license or his removal from the list of authorized physicians. In addition, testimony and/or depositions concerning independent medical examinations conducted through December 4, 2007, may be provided.
