Date: January 23, 2008

DOUGLAS W. WOLFF, D.O. (Authorization No. 176406-7W)

Please be advised that, by Determination and Order (BPMC No. 07-280) of a Hearing Committee of the New York State Department of Health, State Board for Professional Medical Conduct, effective December 21, 2007, Dr. Douglas Wolff's license to practice medicine in the State of New York, or in any other jurisdiction where that practice is predicated on a valid New York State license to practice medicine, was revoked.

Dr. Wolff's current address is 316 West 93rd Street, Apt. 5G, New York, New York 10025.

Dr. Wolff is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Wolff for treatment or examinations rendered prior to December 21, 2007, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Wolff regarding treatment or examinations he rendered prior to December 21, 2007, should not be denied due to the revocation of his license or his removal from the list of authorized physicians.

He was removed from the State Education Department's list of licensed physicians, effective July 5, 2007, on which date he was deemed to be no longer registered to practice medicine in New York State.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised his authority to remove Dr. Drakatos from the list of authorized physicians, effective January 8, 2008. Dr. Drakatos's current address is 123 Penny Creek Drive, Bluffton, South Carolina 29909.

Dr. Drakatos is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Drakatos for treatment or examinations rendered prior to July 5, 2007, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Drakatos regarding treatment or examinations he rendered prior to July 5, 2007, should not be denied due to the non-registration status of his medical license or his removal from the list of authorized physicians.
